Does this toner exfoliate like the Clear Pad does?

No. It is purely a hydrating, calming toner at pH 5.5, built on 77% heartleaf extract with hyaluronic acid and centella, and it contains no acids at all. If you want gentle exfoliation, that is the Clear Pad with PHA.

Should I use a cotton pad or my hands?

Either works. Patting it in with clean fingers wastes less product and suits reactive skin; a cotton pad gives you an extra sweep after cleansing. Some people also soak pads and rest them on red patches for a few minutes.

My skin is oily and acne-prone. Will it feel heavy?

It should not. The texture is thin and watery rather than essence-like, it is fragrance-free, and it sinks in within seconds under a light gel moisturiser. It hydrates and calms, but it will not treat active acne on its own.
